COVID Testing this week at GHS
See below for information on rapid testing clinics at GHS this week. Also note that the Curative Testing Kiosk near the Garfield tennis courts is now accepting appointments for qPCR testing: https://book.curative.com/sites/15631
Garfield High School will be conducting student testing mini-clinics this week in the Garfield gym. The testing schedule is:
January 11
7:30 am – 8:35 am
January 12
2:25 pm – 4:00 pm
January 13
7:30 am – 8:35 am
To participate in testing, the following steps should be completed:
- Parents/Guardians should complete a registration form that includes Authorization/Consent.
- Students should come to the gym during one of the testing windows. Students will be asked to share a printout or screen shot of the registration confirmation pop-up at the testing site.
- Students will be provided testing instructions on site.
- Upon the completion of testing, students will be asked to use a QR Code to report results.

Who is encouraged to test?
- Testing is free and insurance is not required.
- All SPS students are encouraged to test.
- Testing is voluntary.

Registration and Consent:
- All sites (except South Shore PreK-8 and Meany) to register and provide consent in advance for all testing sites complete this online registration form.
- You will be asked to share a printout or screen shot of the registration confirmation pop-up when you arrive.
- Translated consent forms will be available at the SPS testing sites or can be printed and completed at home.
Below are the Abbott BinaxNOW rapid antigen test consent forms for students, which can be printed and completed before the testing clinic. Forms will also be available at the school clinic.
