Flu Vaccines Available in Teen Health Center

Flu vaccines for students are now available at the Teen Health Center free of charge. They can come in and speak with Ms. Jahtia for more information on how to sign up for an appointment.

Students need to have a full parental consent and a parent-signed Vaccine Informational Sheet in order to receive any vaccination.

You can print and sign the form below for the vaccines needed. You can also e-mail the Nurse Practitioners (below) and we can contact you via DocuSign secure messaging to obtain your consent.
