Online Student Health Record Portal for Families
Dear families,
Starting Monday, December 3, 2018, all parents and guardians will be able to sign up, view, and update their student’s health information online using the CareDox portal. Access to the CareDox portal will be through the Source. To prepare, be sure you have a working Source account and your student’s school has your correct email address.
In use since September 2017, CareDox is the electronic student health record systems used by district school nurses and other staff to record health information.

For questions about your student’s care, please call the school’s main phone number and ask to speak to the school nurse.
