COVID-19 Vaccines at the Teen Health Center

Garfield Teen Health Center will be starting to provide Covid vaccines (Pfizer) in the next few weeks. We will be offering vaccines on September 30 and October 5. Consent forms are attached. Students can also pick up consent forms at the Teen Health Center. If they are under age 18 a parent will need to sign the consent form before the student’s appointment. If your student is in need of a second dose, please have them bring their Covid vaccine card with them.
